Particularly large and heavy breasts can create both aesthetic and physical concerns for women. While some women are self-conscious about a disproportionately top-heavy figure, other women may experience physical symptoms as well. Heavy and pendulous breasts can cause back, neck, and shoulder pain. Additionally, when heavy breasts rest on the chest, moisture may be trapped, leading to chronic skin rashes and infections underneath the breasts. A breast reduction always includes a mastopexy, to help lift, rejuvenate and reshape the breasts in order to achieve an optimal result. The surgery can also be combined with other aesthetic procedures, such as  liposuction of the chest wall or back fat (commonly referred to bra roll).

What is Breast Reduction Surgery?

A breast reduction surgery aims to reduce the overall size of the breasts. This is achieved by removing tissue, skin and fat from the breasts to relieve patients from discomfort and obtain a more desirable appearance. A breast reduction surgery also involves repositioning the nipple and reducing the size of the areola. 

What Can I Expect During A Consultation?

In assessing you as a candidate for breast reduction, your doctor will carefully check:

  • The size and shape of your breasts
  • The quality of your skin, and
  • The placement of the nipple-areola complex.
  • Current medications, vitamins, herbal supplements, alcohol, tobacco and drug use
  • Previous surgeries

Dr. Peter will also:

  • Evaluate your general health status and any pre-existing health conditions or risk factors
  • Take photographs
  • Discuss your options
  • Discuss likely outcomes of arm lift surgery and any risks or potential complications

If you are overweight, your plastic surgeon may suggest that you lose weight before having surgery.

Breast Reduction Procedure

Your surgery is performed under general anesthesia on an outpatient basis. It takes 3 hours to perform a breast reduction.

An anchor-shaped incision is carefully made in your breast: around the areola, down to the crease under your breast, and along the crease. Excess tissue, fat and skin are removed. Liposuction can effectively remove fat in hard to reach places, if necessary. Your nipple and areola are moved to a higher position, giving your breasts a perkier appearance, and the remaining skin is pulled down to add an improved tone and shape.

Can it be combined with other procedures?

Yes. Breast reduction surgery is often combined with breast lift surgery in order to reduce the size and weight of breasts, while improving the shape and position of the breasts. Some patients choose to combine breast reduction with other cosmetic procedures, such as inverted nipple correction, tummy tuck, and liposuction. Dr. Peter Biribwa will discuss with you what procedure or combination of procedures may best suit your specific needs giving your breasts a perkier appearance, and the remaining skin is pulled down to add an improved tone and shape.

Are You a Good Candidate?

You may be a candidate for breast reduction if:

  • You have large breasts that affect your daily activities
  • You have large breasts that are causing symptoms
  • The weight of your breasts causes grooves in your shoulders from your bra
  • You have neck or back pain related to strain from heavy breasts
  • Your breasts are too large for your preference
  • You want to correct droopy breasts and also want them smaller

How to Prepare for Breast Reduction Surgery

In preparing for surgery, you may be asked to:

  • Get lab testing or a medical evaluation
  • Take certain medications or adjust your current medications
  • Stop smoking
  • Avoid taking aspirin, anti-inflammatory drugs and herbal supplements as they can increase bleeding

Post-Operative Instructions at a Glance

 
  • Pain medication and antibiotics are prescribed for the first week following surgery. Any post-operative pain is typically mild to moderate and should be taken as prescribed.

  • You will find tensors on your chest after surgery which should be worn for the first week even while sleeping.  They can be switched to a sports bra afterwards.

  • Patients are able to walk after surgery.

  • Incisions are to be kept dry for the first week. Patients can shower after that.

  • Patients are able to return to light duties at work typically after 2 weeks.

  • Ends of dissolving sutures are trimmed at 3 weeks.

  • Patients are able to resume full activities and to exercise after 6 weeks.

  • Immersion of incisions in baths, hot tubs, pools, oceans should be avoided for 6 weeks.

 

What Can I Expect in My Breast Reduction Surgery Recovery?

 

Breast reduction surgery requires some recovery time before you can resume your normal, day-to-day activities. Immediately following the surgery, there are restrictions as to the amount and level of your activity. Most women are able to return to work and engage in light activities after 1-2 weeks.  You should avoid bending over or lifting objects above 10 kgs for roughly another 4 weeks.  The total recovery time after breast reduction surgery is generally 4 – 6 weeks.

The recovery process also requires a follow-up appointment that will be scheduled at 1, 3 and 6 weeks after the surgery for continual management and post-operative care.

It may take up to a year for your breasts to “settle” into their new shape. Please be aware that hormones, weight gain or loss and pregnancy can cause their shape to fluctuate.

What Will My Breast Reduction Scars Look Like?

As with any surgical procedure, there will be some residual scarring from your breast reduction surgery. However, our plastic surgeons take every precaution to keep incision lines as concealed as possible. Typically, Breast reduction leaves noticeable, permanent scars in an anchor shape on your breasts. They can be covered up by a bra or bathing suit.

How Long Will the Results of My Breast Reduction surgery Last?

Many of our patients remain in contact with us well after they’ve recovered from surgery. They tell us how they’re doing or to schedule another, unrelated procedure — but not for a revision breast reduction. This is because at Ace Plastic Surgery, we believe that your breast reduction results should last as long as possible. For this reason, we don’t apply surgical techniques for “mini” breast reductions that achieve only temporary, short lived results. We perform techniques that have been medically researched and proven to reduce your breast size, permanently.

What Risks Are Associated with Breast Reduction Surgery?

All surgery carries some uncertainty and risk, but we have extensive experience performing breast reduction which ensures that complications are extremely rare. Post-operative complications include:

  • Bleeding and infection, which can cause scars to widen
  • Poor healing and scars (more common in smokers)
  • Unevenly positioned nipples
  • Loss of feeling in nipples and breasts (which can be permanent in rare cases)
  • Very rarely, the nipple and areola can lose their blood supply and the tissue will die. They can be rebuilt using skin grafts from elsewhere on the body, but will no longer have much sensation.

Although the nipples stay attached to the breast tissue during surgery, sensation can be affected. And while breastfeeding may still be possible, there is no guarantee that you will be able to breastfeed after the surgery. This is because the surgery removes many of the milk ducts leading to the nipples. If you’re planning to nurse children in the future, you should wait until after that’s been completed to have breast reduction surgery.

There is a higher risk of these complications in patients who are smokers, are diabetic or have poor circulation. You can reduce your risks by following the care instructions we will provide you with before your surgery, resuming activity after surgery, and by not smoking.

Dr. Peter Biribwa Kansiime is the founder and Director of Ace Plastic Surgery clinic specializing in Plastic and reconstructive surgery at the Coast Of Kenya.

Dr. Peter Biribwa Kansiime is a is a double board certified licenced plastic surgeon in Kenya and Uganda, practising Plastic, Reconstructive and Aesthetic surgery. He got his medical degree from the University of Nairobi.
After serving as an intern and resident at the Kenyatta National Hospital, Aga Khan Hospital among other hospitals in Kenya, Dr Biribwa completed his residency in Plastics, Aesthetics and reconstructive surgery at the University of Nairobi.
He has also undertaken a course in Microsurgery at Sefako Makgatho University in South Africa (Medical University Of South Africa – MEDUNSA)
